I have provided everything requested, and yet, they claim they need the information. It is ON THE RECEIPT ITSELF.
It is ridiculous the run around they give you. Unfortunately, I am "stuck" with them, as my employer only offers Kaiser as the alternative, and we do not live in a Kaiser area.
(NOT THAT I WANT THEM, Either!)
I have called no less than 5 times, emailed the specialty person at least 10 times, filed a complaint with the Insurance Commissioner, and have filed a complaint with the Attorney General's office. (These are recent, so no response as of yet.) It will be interesting to see if they respond or act!
